1. BOX-LIC-032-E which includes:
. Authorization to release medical records
. The comprehensive medical examination must also include proof that within the last six months, the applicant has been tested for and is free of the Hepatitis C virus antibody and the human immunodeficiency virus
(HIV)antibody, and that the applicant is not acutely or chronically infected with the Hepatitis B virus by testing the Hepatitis B surface antigen.
. Proof of having passed an ophthalmologist medical examination (eye exam) within the previous six months
. If you are 36 & older, you must submit a favorable EKG & EEG
2. Contestants 17 years old (not yet 18 years old) must provide a notarized Parent Consent Form Box-035-N-E.
3. Drug Test Authorization Form BOX-034-N-E.
