Search the Doctors of Podiatric Medicine with Electronic Prescribing Waivers database to verify an electronic prescription waiver.

Update Your Physician Profile

If you need to change your mailing address, phone number, or email address, please log into the Online Licensing System.  Once logged into your account, select Manage your License, then submit the updated contact information and save the changes.

Order a Duplicate License

You can request a duplicate license by logging into the Online Licensing System.  Once logged into your account, select Request Duplicate License. The fee to request a copy of your license is $25.

Duplicate Renewal Receipt

You can request a copy by logging into the Online Licensing System.  Once logged into your account, select Request Renewal Receipt. The fee to request a copy of your license is $25.
