Licenses are valid for a period of two years from the date issued.
To report a change in address, please submit a completed Court-Ordered Program Application for Address Change (PDF) with the application fee of $25.
Renewal of Expired Licenses
You may not operate a court-ordered program school if your license has expired.
A program provider that fails to submit a completed renewal application and pay the renewal fee before the expiration date of their certification will no longer be certified to provide the applicable educational program.
Expired licenses may not be renewed. The program provider must reapply and submit a completed Court-Ordered Program Application (PDF) with the application fee of $300 as required under §90.31 to obtain a license as an approved court-ordered program provider.
A court-ordered program provider may not offer, provide or operate a court-ordered program with an expired license.
If a program provider license expires, then any branch site licenses to the program provider also expire. Court-ordered programs may not be operated at branch sites if the main program provider license has expired.
Active Instructors
Providers are required to have at least one active instructor on their roster at all times.
